Paul Sheldon, a renowned author of romance novels, finds himself stranded in the middle of nowhere, Colorado, after his car crashes in a blizzard. Injuries sustain his back and ankle and leave him temporarily paralyzed. In desperation, he calls for help, but the nearest hospital is too far away for a rescue team to reach in the short time it'd take. Luckily, Annie Wilkes, a seemingly kind-hearted and elderly woman, comes to his aid, nursing him back to health in her isolated farmhouse. What starts as a benevolent gesture gradually becomes a descent into madness. As Sheldon begins to regain mobility, he becomes increasingly aware of Annie's unsettling behavior and the disturbing fascination she harbors for his work. Annie confesses to being a devoted fan of Paul Sheldon's romance novels, particularly the Misery Chastain series, which she identifies with due to similarities between the character and her own experiences. Annie's fixation, however, crosses the line into obsession when she reveals she's been taking morphine to alleviate her own chronic back pain.
