The film begins on a sunny, open road in Germany, where two young American women, Jenny and Lindsay, embark on a fun-filled road trip across Europe. Their laughter and carefree banter fill the air as they drive through the picturesque landscape. However, their joyride takes an unexpected turn when their car suddenly breaks down in the middle of the woods, leaving them stranded at dusk. As night begins to fall, the duo becomes increasingly desperate to find help. With no cell phone signal and a damaged vehicle, they set off to find assistance, stumbling upon a nearby villa that appears to be abandoned. Despite initial reservations, the two women cautiously enter the mansion, hoping to find shelter and possibly a way to repair their car. In the dimly lit interior, they meet the enigmatic and disturbing Count Henckel von Donnersmarck, a retired surgeon with a peculiar and unnerving demeanor. He appears at first as a somewhat friendly and helpful figure but quickly reveals his sinister intentions. Donnersmarck is a man consumed by his twisted vision, obsessed with the possibility of creating a human centipede – a being comprised of three individuals connected mouth-to-anus. Donnersmarck's fascination with this concept is rooted in his own past, where he was forced to perform surgery on prisoners of war during World War II. The Count's past trauma and fixation on human suffering drive his macabre desires. He presents to the Americans a chilling and inhumane plan: to create a human centipede, connected via surgical stitches, and have Lindsay and Jenny as key components of this monstrous being. The two women are initially resistant to Donnersmarck's proposition, but he uses persuasion, manipulation, and coercion to convince them that they are safer with him than on their own in the woods. As the night wears on, they are forced to endure an unimaginable fate: being prepared for surgery by the deranged surgeon. The Count's intentions come to fruition when he anesthetizes Lindsay and Jenny, preparing them for the gruesome procedure. Donnersmarck proceeds to surgically connect them to a Japanese woman, Katsuro, who was initially kept captive by the surgeon as part of his twisted experiments. The creation of the human centipede is a harrowing experience for all three women, who must endure unimaginable pain and humiliation as they are sewn together mouth-to-anus. As the three women come to terms with their new reality, they realize that their freedom is long gone.
