Throughout all the dream sequences, Cobb is wearing his wedding ring. Conversely, in all the scenes that are reality, Cobb is without his ring. Therefore, the spinning top is merely a distraction; the ring is the true totem. In the end, Cobb doesn't care whether the top stops spinning or not because he wasn't wearing his ring when he exited the airport. He knew all along that he had returned to reality.
